Dear SOCC Parents and Guardians,
Each year, SOCC participants in grades 3 and 4 are asked to complete a non-academic student survey made available through the New Hampshire Department of Education evaluating 21st Century Community Learning Center afterschool programs. Student survey responses help improve the SOCC’s afterschool program and the experience of participating youth.
Student participation in survey completion is voluntary. Attached is a sample of the survey that students will be asked to complete. A copy of this letter and the survey are also available on the SOCC’s website (www.dhasocc.org). The voluntary survey will be administered beginning April 9, 2019 through April 24, 2019.
Per RSA 186:11, parents and legal guardians must provide written consent for their student to participate in the survey. Please reply via email with your written authorization for participation or see Trevor Martin or Stacey Kearns during SOCC hours at Woodman Park School to sign by April 8, 2019 to utilize a provided form.

The Elementary School program is located at Woodman Park School from 3:05 to 6:00 pm Monday through Friday. The SOCC Afterschool Program is supported by NH Department of Education's 21st Century Community Learning Centers.
